Surgery for the acute dissections of the ascending aorta and the arch

چکیده
Methods From January 2008 to December 2012, 227 patients (168 men, 59 women) aged 12 to 84 years (mean 58) underwent emergency operation for type A aortic dissection. Mean admission-to-table time was 2.3 (1-19) hours. Cannulation was accomplished in 211(92.9%) patients through the femoral artery. Supracommissural replacement of the ascending aorta was applied to 146 (64.3%) patients. In 52 patients (22.9%), the aortic valve was replaced either independently (11 pts, 4.8%) or by means of a composite graft (35 pts, 15.4%). “Open distal anastomosis” strategy was applied in 63 patients (27.8%) with isolated replacement of the ascending aorta, 47 pts (20.7%) with transverse arch, and 10 pts (4.4%) with total arch replacement, 3 (1.3%) of whom had an “elephant trunk”graft extension.

Aortic Pseudo aneurysm 3 Years after Interposition Tube Graft for Acute Dissection of Ascending Aorta
Introduction: Patients with primary acute aortic dissection are at higher risk of complications, including increasing aortic aneurysm diameter, aortic rupture, aortic pseudo aneurysm, and recurrent aortic dissection. Case presentation We presented the case of a recurrent pseudo aneurysm and rupture of the aorta in the distal a...
متن کاملLong-Term Changes in the Distal Aorta after Aortic Arch Replacement in Acute DeBakey Type I Aortic Dissection
BACKGROUND We analyzed the long-term results of ascending aortic replacement and arch aortic replacement in acute DeBakey type I aortic dissections to measure the differences in the distal aortic changes with extension of the aortic replacement. METHODS We reviewed 142 cases of acute DeBakey type I aortic dissections (1996-2015). Seventy percent of the cases were ascending aortic replacements...
متن کاملThe efficacy and short-term results of hybrid thoracic endovascular repair into the ascending aorta for aortic arch pathologies.
OBJECTIVES Conventional total aortic arch repair is a high-risk procedure, particularly for high-risk patients. Although endovascular treatment of aortic arch aneurysm is a recently induced procedure, only a few cases are indicated and outcomes are questionable. Here, we report on the early and short-term results of our surgical procedure, i.e. hybrid arch repair with supra-aortic debranching a...
متن کاملSun's procedure for chronic type A aortic dissection: total arch replacement using a tetrafurcate graft with stented elephant trunk implantation.
The Sun’s procedure is a surgical technique that integrates total arch replacement using a tetrafurcated graft with implantation of a special stented graft in the descending aorta, as a treatment option for extensive dissections or aneurysms involving the ascending aorta, aortic arch and descending aorta (1-3). To illustrate our technique for performing the Sun’s procedure (1,2), we present a v...
متن کاملInter-arm and Inter-leg Systolic Blood Pressure Differences in Aortic Dissection
The essential feature of an aortic dissection is a tear in the intimal layer of the aorta, followed by formation and propagation of a subintimal hematoma. It is a medical emergency with a high mortality rate if appropriate management is not instituted immediately. There are two different systems classifying the aortic dissections. The Stanford classification divides dissections into two types: ...
متن کاملDebranching Solutions in Endografting for Complex Thoracic Aortic Dissections
BACKGROUND Conventional surgical repair of thoracic aortic dissections is a challenge due to mortality and morbidity risks. OBJECTIVES We analyzed our experience in hybrid aortic arch repair for complex dissections of the aortic arch. METHODS Between 2009 and 2013, 18 patients (the mean age of 67 ± 8 years-old) underwent hybrid aortic arch repair. The procedural strategy was determined on t...
